NHS North Yorkshire and York are committed to ensuring high quality patient focussed services to support individuals who are in the last 6 to 12 months of life, and their carers. Patients, as far as possible, are offered care in their own home in this period of their life, and in particular if this is where they wish to be at the end of their life. The PCT is working towards improved community services to support people in their own homes, and works with district nurses, palliative care teams and other community services, hospices, nursing homes and hospitals to ensure patients receive appropriate care in which ever care settings they choose to be in, or need to be in, as they approach the end of life. We are working with our partners, North Yorkshire County Council and City of York Council, towards seamless health and social care services for those who wish to be in their own home in the last 6 to 12 months of life. Hospices, nursing homes, national and local charities, voluntary services and Patient and Carer groups contribute to the development of improved services for North Yorkshire and York.
