Clinical CommunityLevels of CareThe North Yorkshire and York Levels of Care model is a commissioning tool, which describes what care should be provided to people with a determined level of need in four distinct settings: 1 acute care 2 sub-acute care 3 facility-based intermediate care 4 home-based intermediate care Click here to see a graphic version of the model. It is based on the MCAP (medical care Appropriateness protocols) evidence base, drawn from a wide range of sources. Click here to read more about MCAP Criteria: http://www.theoakgroup.co.uk/mcapCriteria.html What does Levels of Care do for patients? Levels of Care criteria is based on people's needs in terms of medical input, nursing input, rehabilitation, personal care and access to diagnostics. The principle behind commissioning against defined levels of care is to ensure that people receive treatment and care in the most appropriate and least dependent setting, and that people move to a less dependent setting to complete more extended periods of treatment, recovery or rehabilitation (as long as this is their best interests). Why adopt Levels of Care? The Levels of Care model will allow commissioners to ask fundamental questions about whether the community services they commission are organised:
