Diet sheets for health professionals
Department
of Nutrition and Dietetics
The following
range of diet sheets has been designed for first line advice for
patients. They are intended for general use to be issued by GPs,
Consultants, District, Practice and Ward Nurses, Allied Health
Professionals and Registered Nurses in nursing and residential homes.
Through accessing these information sheets you are agreeing to our terms
and conditions of use as follows:

They must only be issued by health care
professionals mentioned above.

They are designed only for use in the
Scarborough, Whitby and Ryedale locality of the North Yorkshire and
York
-
Only give to patients who are not currently
seeing a dietitian. However, this information can be given to those
who are to be referred as first line advice
-
Patients with more complex health issues
affecting their nutritional status should be referred for more
specific dietary advice, e.g. renal impairment, cardiac problems,
gastrointestinal disorders
-
They should be printed on an individual
patient basis so that the most up-to-date information is issued and
hard copies should not be stockpiled.
