Clinical CommunityDo not attempt Cardio-Pulmonary Resuscitation (DNACPR)
The DNACPR form is designed to be transferable between all care settings and to be held by the patient. It is the responsibility of the receiving clinician in charge of patient care to review the DNACPR form as quickly as possible following a change of care setting. The form carries a clear message that it is related to cardio-pulmonary resuscitation only. This ensures patients known to be approaching the end of life receive appropriate care and treatment, and are not resuscitated inappropriately or against their wishes. It also takes into consideration the Mental Capacity Act (2005) and gives a legitimate framework in which to discuss cardio-pulmonary resuscitation at the end of life. Resources Please find below the following downloadable resources: If you are a member of staff within Community or Mental Health Services or work in a GP practice, you can also access via our Intranet site*: * Please note this is only available to staff who are using a computer connected to an NHS network (n3).
